Abstract : Eighty-six incidents (Deviation/Difficulties) involving the Atlas MA-3 propulsion system were reported as part of an operational system testAND EVALUATION PROGRAM CONDUCTED AT Vandenberg Air Force Base, California. Knowledge of these incidents was obtained by direct observation of missile ass mbly and maintenance shops preparation and missile checkout job operations. Fiftyfive human factors problems were identified and analyzed in relation to safety, training, organizational controls, technical data, personnel selection and manning, and the design of engine hardware, aerospace ground equipment, maintenance shop facilities, and missile hardware.
