Abstract : An increase in the number of people with chronic medical conditions who seek care from the health care industry has occurred during the past few years. As a response to the needs of these patients, the AFMS offers inpatient and outpatient care. Unfortunately, the AFMS is oriented toward inpatient and outpatient care as a discrete, acute illness modality. There appears to be a schism where acute care is offered and chronic care is required. By chronic care is meant a flexible mix of health and social services that allows the patient to function independently in his/her activities of daily living. This flexible mix lies beyond the alternatives of hospitalization and outpatient care. It can be called Home Health Care. There is no formal Home Health Care Program in the AFMS. It is the purpose of this problem-solving project to determine whether there is a need for Home Health Care as a component of the services now provided in the AFMS facility. There are numerous chronic medical conditions among patients eligible for care from the AFMS. In this paper, the adult patient with a diagnosis of Diabetes Mellitus will serve as a representative of chronic medical conditions.
