Abstract : The purpose of this study is to determine whether efficiency and cost-effectiveness of providing bone marrow transplantation (BMT) services will be improved through consolidation of the BMT units of Brooke Army Medical Center (BAMC) and Wilford Hall Medical Center (WHMC). BAMC BMT Unit performed 41 BMT during fiscal year 1995 (FY95) that included 31 adult autologous, 6 pediatric autologous, and 4 pediatric allogeneic transplant patients. Average length of stay (ALOS) was 41 days for adult autologous, 65 days for pediatric autologous, and 72 days for pediatric allogeneic. Average total cost per case was $77,603. WHMC BMT Unit performed 57 BMT during FY95 consisting of 30 adult allogeneic and 27 adult autologous patients. Average total cost per case was $65,520 with ALOS of 22 days for adult-autologous and 36 days for adult allogeneic. This study supports the recommendation that BMT units at both BAMC and WHMC should be retained. They provide BMT treatment to patients at much less cost to the government than if those cases were treated in civilian hospitals. The study shows that the presence of two BMT units may enhance patient access and decrease the need for referrals into the civilian sector.
