Abstract : The irreparably damaged ischemic skin characteristic of third degree burns is highly susceptible to infection and this, together with the lowered immunologic defenses of the severely burned patient, increases the danger of sepsis. We, like a number of others (but not all) responsible for the care of severely burned patients, believe the prompt removal of partial-thickness and full-thickness eschar devitalized as the result of the burn followed by immediate or very prompt coverage, ideally with viable, compatible skin, with diminish sepsis, lessen functional abnormalities, and disfigurement, speed recovery, and decrease mortality. We recognize the paucity of data based on prospective randomized clinical studies to prove this view. Nevertheless, it is our belief that this view is correct. The problem is how to do this safety, quickly, and effectively. Accurate diagnosis of depth of burn is important for surgical escharectomy.
